Emergency Contact Forms:

The emergency contact form must be completed and kept up to date. If your phone number/s change, or any emergency contacts change you must notify the Pre-k office immediately at (516) 767-4951.

  • Attendance is important for your child to receive the full benefit of going to school and being involved with the pre-k program.
  • An illness, a doctor’s appointment, or death in the family are considered excused absences.
  • Absences for any other reason are considered unexcused absences and should be discussed with a member of the support staff.

Please continue to fill out the health screening daily. If your child is not coming to school due to illness, please call the Pre-k Nurse’s office at (516) 767-4952 on the first day of the absence.
