Pre-K Emergency and Medical Information
Emergency Contact Forms:
The emergency contact form must be completed and kept up to date. If your phone number/s change, or any emergency contacts change you must notify the Pre-k office immediately at (516) 767-4951.
Attendance:
- Attendance is important for your child to receive the full benefit of going to school and being involved with the pre-k program.
- An illness, a doctor’s appointment, or death in the family are considered excused absences.
- Absences for any other reason are considered unexcused absences and should be discussed with a member of the support staff.
Illness:
Please continue to fill out the health screening daily. If your child is not coming to school due to illness, please call the Pre-k Nurse’s office at (516) 767-4952 on the first day of the absence.
EDLIO:
This is a telephone messaging service set up by the school district. The service will allow the school district to communicate with the families about emergency situations, school events and important issues. The system will reach you at home, work, or your cellular phones within a few minutes of the message going out.
*To ensure you receive these messages, please let us know immediately of any changes in contact/guardian/emergency contact telephone numbers.
School Closings:
In the event of inclement weather, please visit the district website at www.portnet.org. *A telephone message will also be sent to your cell or home phone.
